Introduction

The migration of patient record from conventional towards an IT-based system have enable Putrajaya Hospital to give better patient care and also create new opportunities and challenges for the hospital service delivery.

In relation to the THIS adopted by the hospital, the role of the Medical Record Department (MRD) shall change significantly as records are now in digitized form. MRD will now have to ensure that the safety and functionality of the record are in accordance with the ministry’s guideline.

Facilities and Services

1. Medical Report Application:

Management of medical report application is the front role of this department. Medical report only will be processed by a formal application from patient, relatives, employer etc. Medical report is required for insurance claim, compensation, court proceedings, medical board examination etc.

Basic requirement for medical report application are:

  1. Official application by letter (for application through post) or designated application form (for walk-in application)
  2. Patient’s and treatment’s particular
  3. Consent letter (if application comes from other than patient themself)
  4. Payment based on the number and type of the medical report:

    • RM 40.00 for report prepared by medical officer
    • RM 80.00 for report prepared by specialist
    • RM 200.00 – RM 1000.00 for detailed report by specialist
 

(Medical report charges for foreigners is double)

The time frame for the report to be ready is within 4 weeks.

2. Management of hospital’s statistic:

The healthcare statistics are generated and managed electronically. MRD is responsible for the compilation of the report. MRD thus must ensure that the statistical reports are in accordance with the ministry’s guideline.

3. The management of patient records:

The physical patient records are part of the electronic record which still has to be kept due to the legality of the document and practicality for future references. These records are available for borrowing by doctors, for research purposes and litigation case. The department shall keep the records for the length dictated by national policy.
